Answer 2 :

Start heparin combine with warfarin for 4-5 days until INR = 2-3
(unfractionated heparin 80unit/kg IV bolus then 18unit/kg/hr or LMWH (enoxaparin 1mg/kg) q 12 hr)
2.continue warfarin 3-6months , if there is recurrent of DVT, warfarin should be continue for 1 year.
