Good answer, that seem to have more evidence of venous thrombosis, and you also give the other causes of chronic legs swelling. Do not to forget to find out “what are the risk factors of this case; such history of taking hormonal drugs, history of surgery and other family members with thrombosis.

Risk factors showed no significant, but do not forget to find out others history such as history of taking hormonal drugs, history of surgery and other family members with thrombosis. This may suggest giving us the cause of thrombosis event.

Sahara, your answer #1 is correct, this urine color is red with babbles. This can see in intravascular hemolysis patients.
But answer #2 the shape of RBCs is leakage out content, and contracted red blood cells showed.

This is a patient with G6PD deficiency.

Urine color is red and frothy are correct.
I would like to add more information of blood smear that; red blood cells content contracted on one side. This call contracted red blood cells with some leakage out of red blood cells. Spike membrane is leakage of red blood cells content.

this is a patients with G6PD deficiency.

ดีมากครับ ที่เก็บรายละเอียดจาก case แล้วมาแปลเป็นข้อมูลทั้ง Positive และ negative findings
ส่วนประเด็นการส่งตรวจ Chromosome เนื่องจาก โรคที่เราคิดถึงได้อีกโรคคือ Myelodysplastic syndrome (MDS) ที่สามารถพบได้ใน Case ผู้ป่วยสูงอายุที่มีภาวะโลหิตจาง โดยเมื่อเจาะไขกระดูกได้มีการตรวจ Chromosome study ในกรณีนี้ผลตรวจ Chromosome study= 46,XX
จะเห็นได้ว่าใน Case นี้ ได้มีตั้งคำถามโยงกับการเก็บสิ่งส่งตรวจที่เหมาะสม ครับ

ดีมากครับ พัสกร ที่พยายามคิดเป็นระบบ และหาข้อมูลที่ได้จาก case
น่าสนใจในกรณี Fanconi anemia จะพบในผู้ป่วยเด็ก มีความผิดปกติทางกระดูก ร่วมด้วยครับ
และอีกประเด็นที่น่าสนใจ คือการให้ยารักษา ในกรณีที่มี Vitamin B12 deficiency ต้องทำการรักษาก่อนเพื่อให้ ความผิดปกติทางระบบประสาทกลับมาเป็นปกติมากที่สุด
